The Unified Solution
for Skilled Nursing
One entry. Real-time visibility.
Built to close the gap while it still matters.
Built for the realities of skilled nursing.
Prysm exists to give skilled nursing facilities the tools they’ve always deserved, software that actually works. We believe that when documentation is effortless, compliance is automatic, and operators have real-time visibility, nurses can do what they came to do: care for people.
Solution
Every feature in Prysm is
designed around how skilled nursing actually works.
Unified Documentation
One entry drives nursing notes, care plans, tasks, and interdisciplinary outputs.
Compliance at Entry
Validation runs at the point of documentation, not during an audit.
MDS & PDPM
Capture IPAs, track ARDs, and keep reimbursement accurate with built-in logic.
Operator Command Center
Census, staffing, documentation status, and task completion in real time.
Automatic Escalation
Problems escalate to the right person while there is still time to act.
Facility-Wide Visibility
Administrators and operators see census, staffing, documentation, and compliance across their entire facility - all in one place.
About Us
We’ve seen what
broken software costs.
Prysm was founded by people who spent years inside skilled nursing as operators, clinicians, and technologists. We watched nurses spend more time documenting than caring. We watched compliance gaps go unnoticed until a survey. We watched operators make decisions with data that was days old. We built Prysm because we knew it didn’t have to be this way.
We’re building Prysm to replace patchwork systems with a single platform that works the way care actually works.
Careers
Long-term care runs on software nobody would choose today.
It is a large slice of American healthcare and it has been underserved for decades. We are early, well backed, and looking for a small number of engineers who want to work on it.
Remote-first
NYC and Bay Area preferred, with quarterly in-person company events.
Full benefits
Healthcare, dental, vision, and 401(k).
Time in the field
Periodic facility visits. The product is judged by the nurses using it, so you go and watch them use it.
Fully funded
Institutional backing from operators and investors already embedded in long-term care.
Open roles
Senior and staff engineering, remote-first.
